Individuals and families living within the Practice Boundary can register with Keith Health Centre. You will be asked to complete a registration form for each individual wishing to register. Forms are available at the health centre, or can be printed here, completed and brought in to reception.
Register as a Permanent Patient
If you are over 16 years of age who will also be asked to bring two forms of ID (one of which should be photographic), along with proof of residency in this country by way of a utility bill. Although not essential for registering as a patient, we do require this information to register you for online services. You will also be required to fill in a medical questionnaire, and to bring with you up to date details from your previous practice of any medication which you regularly take (including dosage and frequency). This helps us to treat you whilst we await the arrival of your medical records from your previous practice. Please note that if registering your baby, you must bring the form from the Registrar.
Once the registration process has been completed, and we have received your medical records, you will be invited to attend a screening appointment.
